• Provide direct patient care to residents with varying levels of needs, including assisting with activities of daily living (ADLs), administering medications, and maintaining accurate records

• Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the unique needs and goals of each resident

• Monitor and report changes in resident condition to the nursing staff and other healthcare professionals

• Perform routine vital sign checks, wound care, and other medical procedures as needed

• Maintain a safe and clean environment for patients, staff, and visitors

• Participate in quality improvement initiatives to continually improve the delivery of patient care
