Ce poste est à temps complet, avec des quarts de jour allant du 20 octobre 2025 au 3 mars 2025. Vous serez responsable de la mise en œuvre et du suivi de plans d'intervention individualisés. Ce rôle est crucial pour la stabilisation et la réinsertion sociale des usagers, en travaillant en étroite collaboration avec d'autres professionnels.
Key Responsibilities:
• Évaluer les besoins psychosociaux des usagers en santé mentale
• Élaborer et suivre des plans d'intervention individualisés
• Apporter un soutien psychosocial aux familles et groupes
• Collaborer avec les psychologues et intervenants sociaux
• Contribuer à la prévention et réinsertion des usagers
Requirements:
• Baccalauréat en service social ou en psychologie
• Formation OCCI est un plus
• Être formé en IGT, NID-EP
• Membre d'un ordre professionnel nécessaire
• Expérience en intervention souhaitée
