Job Summary
BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full‑time role performing home‑health visits for our office located in Williston, VT. The office serves adult and geriatric clients on a per‑visit basis throughout Franklin County. At least one year of prior clinical experience as a licensed Physical Therapist is required. As a home‑care PT, you will be an integral member of a multidisciplinary health‑care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to remain at home.
Responsibilities- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, such as joint mobility/range, transfer status, balance and safety assessments, and strength/endurance evaluations.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continuously assess and revise care plans, and collaborate with nursing as part of the multidisciplinary plan.
- Educate and instruct clients, family members, or other representatives in rehabilitative care and activities that promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations on a state‑of‑the‑art tablet device.
- Current Vermont PT license.
- Minimum of one year recent work experience as a PT.
- Graduated from an APTA or AMA approved program, verified by transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem‑solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Base Pay: $63–$68 per point depending on qualifications.
