Compensation: $100,000 - $170,000 Annual Salary

Position DescriptionWe are looking for a dedicated Nurse Practitioner to serve as the primary care provider for our skilled nursing facility in St. Albans, VT. This is a full-time, in-person position, Monday through Friday during daytime hours, with no nights, no weekends, and no on-call. You will manage a panel of long-term care and short-stay rehab residents, handling chronic disease management, acute changes in condition, and coordination with the facility's nursing and therapy teams.
Qualificationsof the Nurse Practitioner:
- Active and unrestricted Vermont Nurse Practitioner license in good standing
- Active DEA registration
- Board certification as an FNP, AGNP, or AGPCNP
- Experience in skilled nursing, long-term care, or geriatrics preferred
- Provide primary care to residents, including physical exams, assessments, and medication management
- Diagnose and treat chronic and acute conditions within the NP scope of practice
- Order and interpret labs and diagnostic tests, and adjust care plans accordingly
- Collaborate with facility nursing staff, therapy, and the interdisciplinary team
- Maintain accurate and compliant clinical documentation
