LVN​/LPN – LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in long-term care or skilled nursing facilities. Ensure patient safety, comfort, and compliance with healthcare standards through effective clinical practices.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor residents’ conditions and report any changes to healthcare team.
• Document patient assessments, interventions, and progress in accordance with facility protocols.
• Assist residents with activities of daily living to promote independence and dignity.
• Follow infection control policies and ensure a safe environment for patients and staff.
• Collaborate with interdisciplinary team to develop and update individualized care plans.

Qualifications:

• Valid state licensure as LVN or LPN.
• Minimum of 1 year of experience in LTC or SNF settings preferred.
• Demonstrate knowledge of medical terminology, nursing practices, and safety protocols.
• Exhibit strong communication and interpersonal skills.
• Complete necessary training including COVID-19 vaccination or approved exemption documentation.
• Ability to work weekends and flexible schedules as required.
