LVN​/LPN – LTC​/SNF

Job Summary:
Seeking an experienced Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a local contract position in a long-term care or skilled nursing facility. The role involves providing quality nursing care to residents and supporting the healthcare team to ensure optimal patient outcomes.

Responsibilities:

• Administer medications and treatments in accordance with healthcare plans.
• Monitor residents’ health status and promptly report any changes or concerns.
• Support with wound care, vital signs, and other basic nursing procedures.
• Document patient care accurately and maintain detailed medical records.
• Collaborate with multidisciplinary team to develop and implement care plans.

Qualifications:

• Valid and active LVN or LPN license in the state.
• Proven clinical experience in LTC/SNF settings.
• Ability to follow detailed instructions and adhere to safety protocols.
• Strong communication and teamwork skills.
• Must bring valid state or federal  wear a photo badge at all times.
• Must have completed required COVID-19 vaccination or have approved exemption documentation.
