Mercy South Hospice in St. Louis is seeking an experienced RN Intake professional to handle Hospice, Home Health and Home Infusion referrals across acute care, physician clinics and identified nursing homes. You will coordinate with discharge planners, social workers and physicians to facilitate timely discharges and ensure smooth patient transitions.
Requirements include an active RN license, 3 years nursing experience with Hospice/Home Health experience preferred. CHPN certification is a plus.
