Home Health Speech Language Pathologist (SLP) to evaluate, diagnose, and treat speech, language, cognitive, and swallowing disorders. Collaborate with an interdisciplinary team for comprehensive care.

• Conduct evaluations and create individualized treatment plans
• Provide therapy for speech articulation, language development, voice, and swallowing disorders
• Educate patients and caregivers on exercises and strategies to support progress at home
• Monitor patient outcomes and update care plans as needed
• Document assessments, visits, and progress accurately in compliance with Medicare and agency standards
• Collaborate closely with nurses, physicians, and other therapists to coordinate care

• Active Florida SLP license (required)
• Home health experience preferred
• Familiarity with Kinnser/Well Sky EMR a plus
• Excellent communication, organization, and time management skills
• Compassionate, patient‑focused, professional demeanor
• Ability to work independently and manage a flexible schedule
