We’re looking for an experienced and compassionate Home Health Physical Therapist to join our growing team. The right candidate will have strong clinical judgment, excellent communication skills, and a heart for patient-centered care.

• Conduct OASIS assessments and initial start-of-care visits to establish individualized plans of care
• Evaluate functional limitations and develop personalized therapy goals and interventions
• Provide lymphedema management and treatment
  , if certified
• Guide patients through therapeutic exercises and mobility programs
• Educate patients and caregivers on home exercise and safety techniques
• Collaborate with physicians, nurses, and other members of the care team
• Monitor patient progress, update care plans, and maintain thorough documentation
• Ensure compliance with all Medicare and home health standards

• Current Florida Physical Therapist (PT) license (required)
• OASIS and start-of-care experience (required)
• Lymphedema certification (preferred but not required)
• Previous home health experience strongly preferred
• Excellent communication, time management, and documentation skills
• A compassionate, dependable, and professional attitude
