OverviewThe Medical Social Worker provides psychosocial support to patients and families, addressing economic, social, and emotional needs within a home health agency setting. This role supports safe and effective care transitions, advocates for patient rights, and contributes to interdisciplinary care planning.
Key Responsibilities- Patient Assessment and Planning – Assess patients and families to identify psychosocial needs related to illness and end-of-life care. Evaluate mental health status, insurance coverage, and financial assistance options. Develop comprehensive discharge plans based on interdisciplinary input and case management principles.
- Interdisciplinary Collaboration – Communicate effectively with physicians and the care team to coordinate services. Participate in regular team meetings and provide on-call social work coverage as needed.
- Discharge Support and Counseling – Facilitate discharge planning from hospitals to home or nursing facilities. Provide counseling and crisis intervention, including guidance on Advanced Directives and bereavement support.
- Documentation and Compliance – Maintain timely and accurate clinical records. Ensure adherence to agency policies, state and federal regulations, and accreditation standards.
- Quality and Community Engagement – Contribute to quality improvement initiatives. Engage in community education to support program growth and strengthen service reach.
- Patient Rights and Safety – Protect patient confidentiality and comply with HIPAA. Promote patient rights and report concerns promptly. Follow infection control and emergency response procedures to maintain a safe care environment.
- Professionalism and Team Support – Collaborate with patients, families, and colleagues. Support staff orientation and ongoing professional development. Maintain clinical competency and serve as a positive role model for teamwork.
- Administrative Responsibilities – Complete required documentation accurately and on time. Coordinate with community resources and contracted providers. Manage time effectively to meet productivity standards while delivering high-quality care.
- Master's Degree in Social Work from an accredited institution
- Minimum one year of experience in a healthcare setting (home health experience preferred)
- Strong communication, documentation, and problem‑solving skills
- Ability to work collaboratively within an interdisciplinary team
- Commitment to patient-centered, safe, and compassionate care
This position involves patient care in home and community settings, with frequent standing, walking, and occasional lifting up to 35 pounds. Daily travel is required. Tasks do not routinely involve exposure to blood or body fluids but may require performance of unplanned tasks where exposure could occur. Employees in this category must be trained in exposure control procedures and provided access to PPE.
