We are hiring a Patient Care Manager with home health experience. As a Patient Care Manager, you will have overall supervision and coordination of clinical services, ensuring high-quality care for home health patients in accordance with physician plans and all applicable laws and regulations.

The Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services. The role requires coordinating and supervising an interdisciplinary team to assure continuity of high-quality care for patients assigned to the team’s area, following physician-prescribed plans of care and all applicable state and federal regulations.

• Supervise and coordinate clinical services, including nursing care in patients’ homes as prescribed by physicians.
• Receive and evaluate referrals, ensure appropriate clinician assignments, and coordinate patient orders and care visits.
• Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Continuously assess patient needs, ensuring care is individualized, appropriate, and compliant with eligibility criteria and physician orders.
• Review assessments and plans of care daily, consult with clinicians, and provide recommendations as appropriate.

• Current RN licensure in the state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance (or access to dependable transportation).
