As a Direct Support Professional
, you will help ensure the safety and well‑being of the individuals we support while assisting them with daily activities, community involvement, and individualized goals. You'll work alongside clinical and program staff to provide consistent, person-centered, and trauma-informed support.

• Provide supervision and support according to each individual's Person-Centered Support Plan.
• Encourage individuals to participate in activities that promote greater independence.
• Assist individuals with activities of daily living based on their specific needs.
• Provide assistance with meal preparation, personal hygiene, toileting, and other daily activities as appropriate.
• Support individuals in achieving goals identified in their program plans.

Community Involvement

• Help individuals participate in community activities and build meaningful connections.
• Provide transportation to individuals as needed.
• Assist individuals with accessing community resources and supports.
• Provide transportation or assistance with medical appointments, Social Security needs, and other community-based services.

Health, Safety & Medication Support

• Administer medications as needed and according to established procedures.
• Promote the safety and well‑being of the individuals receiving services.
• Follow incident management and reporting procedures.
• Provide support using trauma-informed care principles.

Documentation & Teamwork

• Complete required documentation accurately and within established time frames.
• Communicate effectively and respectfully with individuals, families, coworkers, and community partners.
