The Speech-Language Pathologist (SLP) at Back to You Rehab provides high-quality, primarily in-person speech-language therapy services to patients across our locations, with occasional telehealth sessions as clinically appropriate. This role is integral to our interdisciplinary clinical team and focuses on helping patients reach their fullest functional potential while maintaining strict compliance with HIPAA, clinical, and ethical standards.
Key Responsibilities Direct Patient Care- Deliver high-quality, in-person speech-language therapy services in a clinical setting; provide telehealth services when appropriate.
- Conduct comprehensive evaluations for speech, language, cognitive-communication, and swallowing disorders.
- Develop individualized treatment plans with measurable goals and update plans based on patient progress.
- Provide patient- and family-centered education, including evaluation results, treatment plans, progress updates, and home exercise programs.
- Complete accurate, timely documentation in the HIPAA-compliant EMR system.
- Submit all session notes by midnight on the same day of service
. - Complete evaluations, SOAP notes, progress notes, and discharge summaries in accordance with company and payer requirements.
- Generate and submit a Plan of Care (POC) with every evaluation and progress note.
- Fax POCs to referring physicians for signature using the EMR system and ensure continuity of care.
- Complete progress notes for auto insurance patients at least every three months, unless otherwise clinically indicated.
- Monitor and manage treatment schedules for accuracy.
- Follow clinic protocols for cancellations and no-shows, including documenting and applying no-show fees when applicable.
- Request supervisor approval for schedule changes, availability adjustments, or time‑off requests in advance.
- Coordinate care with interdisciplinary team members, including physicians, case managers, attorneys, and other healthcare professionals.
- Participate in team communication to ensure comprehensive, patient-centered care.
- Play a central role in building, expanding, and standardizing Speech‑Language Pathology services across all Back to You Rehab locations.
- Assist in developing clinical programs, specialty services, workflows, and service standards.
- Identify growth opportunities, new referral streams, and expanded service offerings.
- Support onboarding, training, and mentorship of Speech‑Language Pathologists and SLP support staff.
- Develop clinical guidelines, best‑practice protocols, and training materials.
- Foster a culture of learning, clinical excellence, and continuing education.
- Monitor clinical outcomes, documentation quality, and compliance within the SLP program.
- Collaborate with leadership to identify improvement opportunities and implement solutions to enhance patient care and operational efficiency.
- Support marketing and growth initiatives for the SLP program across all Back to You Rehab locations.
- Build and maintain strong relationships with referral sources, including physicians, case managers, attorneys, and community partners.
- Collaborate with leadership on outreach strategies to increase awareness of SLP services.
- Assist with marketing initiatives such as social media content, podcasts, educational materials, and community outreach.
- Represent Back to You Rehab professionally at networking events and community meetings as needed.
- Exhibit professionalism, respect, and courtesy in all interactions with patients, families, and team members.
- Provide unbiased care to patients of all ages, backgrounds, cultures, and abilities.
- Respect patient rights, autonomy, and involvement in treatment decisions.
