The physical therapist is responsible for providing physical therapy services.

• Provides physical therapy services to patients according to a written physician’s plan of care, which may include:
  • Assessing and evaluating therapeutic, rehabilitative, or functional status and contributing to the development of the total plan of care, including evaluation of the home environment.
  • Assessing muscle strength, mobility, gait, ROM, and potential for rehabilitation.
• Directing physical therapy treatment.
• Instructing patients and families/caregivers in the use and care of therapeutic appliances.
• Determining priority needs for physical therapy.
• Reporting to the physician patient’s reaction to treatment or changes in condition.
• Initiating the physical therapy program and instructing other personnel and/or family/caregiver members in certain phases, as well as defining program goals through case conferences.
• May train patient in the use of a prosthetic device.
• Supervising homemaker/home health aides when such personnel participate in patient’s rehabilitation regime.
• Identifying patient and family/caregiver needs for other home‑health services and referring as necessary.
• Preparing and submitting clinical and progress summaries based on the attainment of goals.
• Participating in discharge planning for the patient.
• Providing physical therapy consultation to home families/caregivers when indicated.
• Providing in‑service education programs for nursing agency personnel as needed.
• Participating in peer consultation process.

• Possesses a degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
