Compensation: $225,000 – $250,000 annually

Position SummaryWe are seeking a dedicated Primary Care Physician (MD or DO) to deliver on-site, relationship-driven care to aging adults in senior living communities. This is a high-autonomy, high-impact opportunity to connect with patients and lead care in settings where you’re needed most.
Key Responsibilities- Conduct regular rounding visits in assisted living and memory care environments
- Perform comprehensive medical assessments and manage complex conditions
- Develop personalized care plans in collaboration with patients and families
- Work closely with NPs, PAs, and care teams to ensure seamless coordination
- Educate residents and their loved ones about care options and goals
- Proactively manage chronic conditions to reduce hospital admissions
- Active, unrestricted MD or DO license in Minnesota
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
- Prior experience in senior living, rounding, or hospitalist medicine
- Strong communication and decision-making skills
- Valid driver’s license and auto insurance
