Position SummaryWe’re hiring a Physician Assistant (PA-C) who enjoys being on the move and working in non-traditional clinical settings. You ll serve residents in assisted living and memory care communities, building relationships and delivering meaningful, autonomy-driven care in place.
Responsibilities- Travel to multiple senior living communities weekly to provide medical care
- Conduct assessments, manage chronic conditions, and address acute concerns
- Partner with interdisciplinary teams to align on patient goals
- Maintain accurate, timely EMR documentation and communicate care plans
- Active MN PA license (Board of Medical Practice)
- Background in geriatrics, internal medicine, or complex care preferred
- Strong communication and time management skills
- Must have reliable transportation and current auto insurance
