Nursing Assistant - Community Living Center
Summary – This Nursing Assistant position is located within the VA Community Living Center at the Saint Cloud VA Medical Center in Saint Cloud, MN. Veterans receive nursing home level care including assistance with activities of daily living as well as skilled nursing and medical care. The Nursing Assistant delivers safe, basic physical and emotional care to residents with simple to complex needs while developing technical skills under the supervision of a Registered Nurse or Licensed Practical Nurse.
Base Pay Range – $41,571.00/yr – $67,864.00/yr
Basic Requirements
- United States Citizenship or allowed non‑citizen appointment when qualified citizens cannot be recruited per VA policy.
- English Language Proficiency – basic written and spoken English required.
- Experience – general experience of at least 6 months or 1 year of education above high school with related courses, or completion of an intensive specialized training course of less than one year.
- Qualified for GS‑3 – no additional qualifications beyond the basic requirements.
- Qualified for GS‑4 – one year of experience as a Nursing Assistant or in another medical field, or two years of education related to nursing care.
- Qualified for GS‑5 – one year of progressively responsible assignments at the GS‑4 level or four years of education with health‑care related courses.
- Physical Requirements – extensive periods of standing, walking, bending, lifting, stooping, stretching, and pushing. Fitness, strength, and agility required.
Duties
- Provide direct personal care to residents – bathing, grooming, dressing, toileting, and oral hygiene.
- Assist residents with mobility – transferring, turning, positioning, and ambulation using proper body mechanics and assistive devices.
- Support residents with nutritional needs – serve meals, assist feeding, and monitor intake and output.
- Measure and record vital signs – temperature, pulse, respiration, blood pressure, weight, and oxygen saturation.
- Observe and report changes in residents’ physical or emotional condition to licensed nursing staff promptly.
- Maintain skin integrity – routine skin checks, repositioning residents, and reporting any signs of pressure injuries or breakdown.
- Collect specimens (urine, stool, sputum) as ordered and assist with diagnostic procedures as directed.
- Manage elimination needs – toileting, incontinence care, catheter care, and accurate output documentation.
- Provide respiratory support – positioning residents to promote optimal breathing and assisting with oxygen therapy as directed.
- Ensure a clean, safe, and comfortable environment – infection control protocols and personal protective equipment (PPE).
- Promote resident dignity, privacy, and independence in all aspects of care.
- Document care and resident observations using electronic or paper charting systems.
- Participate in interdisciplinary team meetings and care planning as needed.
- Work Schedule – Full‑time scheduling. Work is scheduled Sunday‑Saturday with rotating day, evening, and night shifts, including rotating weekends and holidays.
