Position OverviewThe Licensed Practical Nurse (LPN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders
- Continuously observe and assess patient condition and care needs, reporting changes to supervisor or physician as appropriate
- Document all activities, assessments, nursing actions, and care coordination in a timely manner
- Participate in, implement, and update the nursing care plan
- Take appropriate nursing action based on assessment and achieve expected outcomes
- Recognize changes in patient needs and respond with appropriate interventions to prevent or reduce risk
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, regulations, and the Nurse Practice Act
- Provide care utilizing infection control measures that protect staff and patient per OSHA standards
- Educate patients and families regarding disease processes, self‑care techniques, and prevention strategies
- Maintain competency knowledge by participating in educational programs, continuing education units, and skill evaluations
- Graduate of an accredited school of nursing
- Current, unrestricted Minnesota state license
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
