• Must have and maintain an active, unencumbered license (RN) in the state where the clinician will practice
• Current CPR certification (hands‑on component) – Aveanna can assist in obtaining this after hire, if necessary
• TB skin test current within last 12 months
• Six months prior hands‑on nursing experience preferred but not required
• Reliable transportation

The Licensed Practical Nurse (LPN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

• Deliver and coordinate quality patient care in compliance with physician orders
• Continuously observe and assess patient condition and care needs, reporting changes to supervisor or physician as appropriate
• Document all activities, assessments, nursing actions, and care coordination in a timely manner
• Participate in, implement, and update the nursing care plan
• Take appropriate nursing action based on assessment and achieve expected outcomes
• Recognize changes in patient needs and respond with appropriate interventions to prevent or reduce risk
• Accept responsibility for personal and professional accountability by complying with Aveanna policies, regulations, and the Nurse Practice Act
• Provide care utilizing infection control measures that protect staff and patient per OSHA standards
• Educate patients and families regarding disease processes, self‑care techniques, and prevention strategies
• Maintain competency knowledge by participating in educational programs, continuing education units, and skill evaluations

• Graduate of an accredited school of nursing
• Current, unrestricted Minnesota state license
• Current CPR certification
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
