Case Manager - Mobile Shelter Support

Join Urban Health and Homelessness Services as a Mobile Shelter Support Program Case Manager in the GTA. Support adults facing complex challenges with tailored case management solutions.

This full-time position focuses on delivering direct case management to individuals in shelters, encampments, and warming centers. You will assist clients in addressing their health, financial, and legal issues through a trauma-informed approach. Building strong relationships with participants and external service providers is essential for helping clients achieve their personal goals.

Key Responsibilities:

• Conduct intake and assessment interviews with participants

• Provide detailed support coordination and advocacy

• Facilitate client engagement initiatives and volunteer programs

• Monitor property issues and liaise with landlords

• Maintain accurate case management and reporting records

Requirements:

• BA/B.S.W with 2 years in human services

• Experience with mental health and substance use challenges

• Commitment to harm reduction and anti-oppression

• Strong crisis intervention and negotiation skills

• Proficient in MS Office and case management tools

Deliver impactful case management support to vulnerable populations while advancing community connections and stability.
