PURPOSE AND SCOPE:
Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients.

Supports the FMCNA commitment to the Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMCNA Quality Goals. This is an entry level MSW role.

PRINCIPAL DUTIES AND RESPONSIBILITIES
  • Patient Assessment / Care Planning / Counseling: assess patients  psychosocial status, collaborate with the care team to identify interventions that meet rehabilitation, treatment goals and improve quality of life; utilize FMC patient education programs, social work theory and methods, and quality of life instruments to address barriers and meet goals; monitor and intervene to help patients adjust to dialysis and achieve optimal psychosocial status;

    provide supportive counseling within scope of clinical training and license; provide educational and goal-directed counseling for transplant; assist with referrals to community resources; maintain knowledge of vocational/educational programs and assist with access to vocational rehab to enable employment or education; assess awareness of advance directives and facilitate access to forms; participate in discussions of DNR status with the team and patient/family;

    adhere to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation; document MSW interactions and interventions.
  • Quality: provide psychosocial support and interventions addressing non-adherence, quality outcomes and quality of life; participate in monthly Quality review meetings and report on indicators related to adherence, quality of life trends, and service recovery.

    Education:

    assess patient knowledge of kidney disease and barriers to adherence; educate patient and family with learning-style appropriate materials; provide information about all treatment modalities; facilitate transplant referral and transplant waitlist management; educate on psychosocial issues related to ESRD and available support services; review patient rights and responsibilities, grievance information and facility policies; collaborate on appropriate QAI activities.
  • Patient Admission and Continuity of Care: review Patient Rights/Responsibilities, Grievance Procedure, Non-discrimination policy, and DNR (if applicable); support scheduling to maximize adherence; interview patient to identify root causes for discharge requests and share concerns with leadership.
  • Insurance and Financial Assistance: liaise with Insurance Coordinators to address insurance issues; educate patients about payment for dialysis and related programs; communicate changes affecting insurance (e.g., transplantation, loss of coverage); refer patients to PBS for billing questions.
  • Staff Related: assist with interview process and hiring decisions as requested; maintain updated patient resource lists (e.g., transportation resources); provide training on psychosocial topics; participate in weekly team huddles and address urgent patient issues with staff.
