Case Manager for Shelter Support Program

Empower individuals facing challenges as a Case Manager at TNG’s Mobile Shelter Support Program in Saint-Joseph-de-Beauce.

Your role will focus on supporting adults in various shelter settings.

This full-time position involves direct support and case management for adults experiencing trauma, homelessness, and related issues in the Greater Toronto Area. By applying trauma-informed practices, you will help clients access vital services and create pathways toward stability and resilience through personalized support.

Key Responsibilities:

• Conduct interviews for intake and assessment purposes

• Coordinate health, legal, and social support services

• Support clients in achieving self-determined goals

• Conduct community-based follow-ups and home visits

• Manage housing property issues and liaise with landlords

Requirements:

• BA/B.S.W plus 2 years experience in human services

• Experience with mental health and substance use issues

• Firm alignment with harm reduction principles

• Excellent communication skills in English

• Familiarity with data management software and systems
