Responsibilities- Conduct resident evaluations, ongoing assessments, and plan therapy interventions.
- Develop and revise care plans based on residents’ needs and establish functional goals, outcomes, and discharge plans.
- Plan for resident discharge and anticipate long‑term and/or short‑term needs.
- Educate the healthcare team, residents, families, and caregivers about the care plan.
- Current state licensure as an Occupational Therapist, in good standing.
- Bachelor’s degree.
