Park Nicollet Health Services is looking to hire an Occupational Therapist to join our Home Care team. As a Partner for Good, you will help make an impact on the care and experience that our patients and their families receive every day.

The Occupational Therapist is responsible for assuring that the PNHS values of Head + Heart, together, are achieved through daily departmental operations. This includes direct patient care, professional growth and development, outcome/compensatory assessments, compliance with regulatory standards, and participating in process improvements to advance patient care outcomes and satisfaction.

The Occupational Therapist will communicate in a timely and professional manner with staff, patients, patient family members, physician, and other internal/external customers. The staff will demonstrate skills in listening, conflict management, system problem solving, decision making and process improvement. Therapists will practice in compliance with professional standards of practice and professional code of ethics.

The Homecare Occupational Therapist acts as a case manager to assess and evaluate patient care needs and provides skilled therapy care and teaching to patients and families in their home. The Occupational Therapist is responsible to improve and prepare patients and families for self-directed and/or assisted care in their community setting. The Occupational Therapist coordinates medical, social, and financial resources to develop a comprehensive plan of care.

The therapist supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery.

Monday – Friday 8:00 a‑4:30 p; occasional Saturdays. Central Region.

• Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy
• MN Occupational Therapy license (MNOTL)
• National Board Certified Occupational Therapy certification (NBCOT)
• Demonstrates knowledge of age‑related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co‑morbidities affecting care.
• Proficiency with communication and data processing equipment, clinical apparatus related to patient care in homecare, electronic medical record system, and complete all OASIS data set except for initial assessment per Medicare regulations.

• Work experience as an Occupational Therapist for at least 3 years.
• Experience in homecare desirable with knowledge of OASIS data set.
• High‑level assessment skill including medication reconciliation and teaching, technical skill, and communication skill.
• Demonstrates initiative, judgement, critical thinking, and ability to problem‑solve independently.
