JOB SUMMARY: Conducts and presents therapeutic recreation programs, assists residents in activities of choice and documents as required by policy.
ESSENTIAL FUNCTIONS:
- Policy and Procedure: Follow all policy and procedures of facility and federal and state regulations. Maintain a professional appearance and attitude and follow designated dress code.
- Documentation: Complete MDS, attend resident care conferences, participate in problem identification, set goals and approaches, review resident goals every 90 days and document changes in medical record, chart as needed when changes occur, maintain daily attendance records, and use computer for documentation.
- Recreation Programming: Encourage active participation of all residents, carry out all groups in a timely manner, assist with creative input in forming of new groups and expanding present ones, and communicate calmly and with respect to staff, residents and families.
OTHER DUTIES: Attend meetings and in-services as assigned by supervisor, and be flexible in the working schedule; weekends, evenings, holidays and special functions are at times required.

High School Diploma
Experience:
Long-term Care and CTRS preferred
