Position Summary

The LPN Care Coordinator supports the Director of Nursing, Assistant Director of Nursing, and RN Nurse Manager in leading and managing a specific nursing unit by assisting with care planning, resource evaluation, and implementation of the nursing process. This role helps collect resident data, contributes to assessments such as MDS and RAPS, develops and updates comprehensive care plans, and collaborates with the interdisciplinary team to ensure resident needs are met.

The LPN Care Coordinator monitors resident conditions, reports changes to RNs, participates in care conferences, and helps ensure care plans reflect current physician orders. Additional responsibilities include providing nursing care under RN supervision, supporting and directing LPNs and nursing assistants, assisting with staffing needs, and ensuring safe, competent care delivery. The position maintains accurate documentation, communicates care needs effectively, adheres to nursing policies and regulatory requirements, and supports quality improvement initiatives.

The LPN Care Coordinator also maintains licensure, participates in ongoing education, and promotes a positive, inclusive, customer-focused workplace culture.

Qualifications
  • Licensure and Certification Requirements:
    Must hold a current LPN license in Minnesota and complete CPR/emergency care certification; must meet employment eligibility per MN Department of Human Services background check.
  • Communication and Functional Ability:
    Must be able to effectively communicate in both verbal and written forms and perform all essential job functions with or without reasonable accommodation.
  • Preferred Experience and Knowledge:
    Experience in healthcare settings like skilled nursing or assisted living is preferred, along with familiarity with Medicare coverage and eligibility.
  • Core Skills and Attributes:
    Requires strong multitasking, adaptability, emotional intelligence, active listening, and the ability to work effectively in fast-paced, diverse, and evolving environments.
