The Registered Nurse (RN), Case Manager – Comprehensive Homecare provides care to clients needing home care services in their home or community setting; develops, implements, and evaluates individual client care plans; and maintains an ongoing responsibility for an assigned caseload.

• Graduate of an accredited school of nursing required.
• Must possess and maintain State of Minnesota RN licensure.
• One year or more clinical nursing experience is preferred.
• Med Surge, TCU/LTC, Home Care, Hospice or public health preferred.
• Possess a valid driver’s license with a good driving record, proof of valid insurance, and reliable transportation.

Pay is within the posted range and is influenced by factors such as skills, education, experience, and market conditions. Shift differentials may increase pay for certain shifts or work.
