Job Summary:
The Physical Therapist (PT) provides intermittent physical therapy to patients in various settings, following physician orders and care plans under the supervision of the Regional Director of Therapy, Outpatient.

Responsibilities:

• Assess patient functional abilities and develop treatment plans.
• Document evaluations and treatment plans for physician approval.
• Coordinate with the interdisciplinary team and supervise Physical Therapy Assistants.
• Instruct patients and caregivers on therapy practices.
• Communicate patient progress and changes to the healthcare team.

Requirements:
Bachelor’s degree in Physical Therapy, valid state license, and preferred two years of experience.

Work Environment:
Indoors in various facilities and patient homes, with travel required.
