• Assess, plan, advocate, and coordinate care from admission to discharge ensuring a safe transition post-hospitalization.
• Maintain patient experience, safety, and quality of care using performance metrics and best practices.
• Implement care management model to provide effective transition planning and length‑of‑stay oversight.

• Master’s degree in Social Work, or completion of all MSW requirements pending conferment (applicable to hires after 12/25/2011).

• 0‑2 years of experience in an acute care hospital setting (minimum preferred).

• Current license in Social Work (LMSW, LCSW) in the state of employment (minimum required).

• BLS (CPR) at hire date required or within 90 days of hire.
• Case Management certification preferred.
