Registered Nurse - Med Surg 9N Bordley - Days at SSM Health

The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care during the patient s hospital stay and coordinates planning with other disciplines utilizing a patient/customer driven approach.

• Performs comprehensive nursing assessment/reassessment.
• Performs age-appropriate admission assessment or transfer assessment and obtains input from family/guardian when appropriate; documents findings.
• Assesses and documents education and discharge needs of patient and family on admission and throughout hospitalization.
• Provides patient reassessment and documents pertinent observations according to the patient plan of care, changes in condition, status and/or diagnosis, response to care, procedures, etc., and standards of care.
• Establishes, coordinates and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols and standards of care.
• Identifies short and long term goals based on patient care needs; formulates nursing interventions to achieve desired patient outcomes.
• Collaborates with health team members for coordination of daily plan of care for assigned patients and communicates patient care with accurate handoffs.
• Administers and documents medications, monitors IV fluids and parenteral nutrition according to policies and procedures.
• Performs referrals as indicated by assessment data and requests consultations for special needs, equipment, or information for patient and/or family.
• Provides patient/family education and discharge planning per documentation guidelines and protocol; clarifies physician orders as warranted and implements them in a timely manner.
• Assists physicians with procedures/treatments as requested or delegates to Care Partner as appropriate; documents Readback for telephone/verbal orders.
• Recognizes changes in patient s condition and takes appropriate nursing actions; involves family/guardian when providing care and in decision-making as appropriate.
• Incorporates infection control practices in daily care and documents/communicates nursing care and patient condition changes.
• Performs ongoing evaluation of care effectiveness, documents patient response to medications, treatments, and procedures, and evaluates and modifies the plan of care as indicated.
• Maintains safety and infection control, adheres to quality standards, and participates in professional development and unit-based improvement activities.

• MINIMUM EDUCATION:
  
  Licensed to work as an RN in the state of Missouri.
• Required Certifications/Licenses:
  Basic Life Support Health Care Provider (BLS HCP) - American Heart Association;
  Registered Nurse (RN) in applicable states/compact as required.
