Overview

The Medical ICU (MICU) at Barnes-Jewish Hospital is a 42-bed intensive care unit that specializes in the care of patients with a full range of clinical issues, including abnormal blood disorders, diabetic keto acidosis, heart, renal and liver failure, GI conditions, infectious diseases, asthma and other conditions. We provide advanced training in critical care interventions such as Swan-Ganz catheters, arterial lines, central lines, ventilators, dialysis, continuous renal replacement therapy and targeted temperature management.

Our orientation and Nurse Residency Program are designed to equip new nurses with the critical thinking and advanced skills needed to be successful in our high-acuity unit.

The unit is led by a Patient Care Manager, Assistant Nurse Managers, Team Lead Charge Nurses, an Advanced Practice Clinical Nurse Specialist, and Clinical Education Specialists. The multidisciplinary team also includes a case manager, social worker, physical and occupational therapists, dietitian, and a pharmacist. The nurse-to-patient ratio is typically 2:1, with some patients requiring 1:1 nursing due to very high acuity.

Role

Purpose

Under the direction of the manager, plans, directs, and coordinates activities of a unit on a designated shift. Performs direct patient care as needed and functions as a clinical resource to nursing and support staff. Assesses, plans, evaluates and documents the nursing process of the assigned unit as needed.

Responsibilities
  • Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting. Critically assesses patient data to proactively mitigate impending changes in condition and establishes consults to the interprofessional team as appropriate.
  • Leads the development and implementation of complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and the patient, family, or caregiver in accordance with established guidelines and standards of nursing care. Plan of care reflects expert holistic clinical knowledge to anticipate and prioritize complex care needs, anticipate barriers, incorporate interprofessional team and caregiver/family input, and identify opportunities to improve patient outcomes.
  • Actively utilizes and champions evidence-based practices (e.g., participates in journal clubs, facilitates practice change based on new knowledge, recommends new standards of care or revisions to current standards, conducts literature searches to support unit goals, disseminates new knowledge from relevant conferences or continuing education).
  • Explores opportunities to advance nursing practice (e.g., support community health events, patient/family education groups, ethics advisory involvement, adjunct faculty, leadership board roles in specialty organizations).
  • Analyzes gaps in practice standards and provides feedback to peers to enhance safe patient care. Leads the evaluation of the patient and family experience and adjusts care to improve satisfaction and quality of care.
