Job SummaryThe Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient’s hospital stay and coordinates planning with other disciplines using a patient‑/customer‑driven approach.
Responsibilities & Requirements- Perform comprehensive nursing assessment/reassessment.
- Obtain age‑appropriate admission or transfer assessment; involve family or guardian as appropriate.
- Accurately document findings and assessment of post‑op/post‑invasive patients.
- Assess and document education and discharge needs of patients and families at admission and throughout hospitalization.
- Provide patient reassessment and document pertinent observations per the patient plan of care, including changes in condition, status, diagnosis, response to care, and procedures.
- Establish, coordinate, and evaluate a plan of care based on assessment data, lab results, physician orders, protocols, and other relevant information.
- Identify short‑ and long‑term goals based on patient care needs and formulate nursing interventions to achieve desired outcomes.
- Incorporate disease‑specific evidence‑based practice into nursing care plans and documentation.
- Collaborate with health‑team members to coordinate daily care plans for assigned patients.
- Communicate accurately during hand‑off reports and patient care coordination.
- Administer and document medications, IV fluids, and parenteral nutrition accurately per policies.
- Complete referrals and request consultations as needed.
- Provide patient/family education and discharge planning per documented guidelines.
- Clarify and confirm physician orders; acknowledge and implement them in a timely manner.
- Assist physicians with procedures or delegate to a caring partner when appropriate; document “Readback” for all telephone orders.
- Recognize changes in patient condition and take appropriate nursing actions, including involving family or guardian in decision‑making.
- Incorporate Infection Control practices in daily care and document changes in patient condition as required.
- Evaluate and document the effectiveness of care and patient/family education, and adjust plans accordingly.
- Report significant changes in clinical parameters immediately to the physician and others as indicated.
- Identify problems, gather data, propose solutions, communicate effectively, and work towards problem resolution.
- Report variations from care or treatment per company policy.
- Provide specialized care to high‑risk patients, including restraint care (initiate and evaluate alternatives, apply restraints as approved, monitor response, provide timely care, consult peers, and document thoroughly).
- Manage pain (assess presence of pain, incorporate cultural beliefs, implement preventive techniques, involve patients or families in the plan, and consider massage, repositioning, and music therapy).
- Assess abuse (recognize criteria, report signs to appropriate departments, and support patient safety).
- Adhere to all quality and performance standards, policies, and procedures while implementing clinical and technical care aspects.
- Participate in learning experiences that increase professional competence and maintain currency with policies.
- Attend staff meetings and committees, keep up with shared governance, and contribute to unit‑based practice councils.
- Assist in orienting/precepting new employees when required.
- Serve as a resource to co‑workers, patients/families, and medical staff; delegate care appropriately and communicate plan of care precisely.
- Supervise and teach students or new staff nurses.
- Maintain safe, clean, and therapeutic environment, practice standard precautions, dispose of hazardous waste, adhesion to hand hygiene, and use safe patient handling techniques.
- Complete annual mandatory requirements (licensure, BLS/HI, TB screening, influenza vaccination, ethics training, safety training, information‑security course, abuse & neglect course, blood‑borne pathogens training, and competency courses as required).
