Additional Information About the Role

Perform professional nursing, consultation, and educational services for persons with diabetes in conjunction with the providers.

• Dayshift Monday-Friday
• 8:00am-4:00pm
• No weekends or holidays!
• Primarily at Christian Hospital, but shared with IL offices as well. Travel will be required a minimum of 1-2 times per week
• Must be licensed in MO & IL

North County Endocrinology and GI Associates is located in North St. Louis County.

Role Purpose

Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development.

Responsibilities

• Monitors patients' response and modifies plan of care accordingly.
• Provides accurate documentation as required.
• Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
• Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care.
• Educates patients and their families on how to manage their condition.

Minimum Requirements

Education

• Nursing Diploma/Associate's
• Nursing/Nutrition/related

• < 2 years
