Clinical Referral Coordinator

This position involves coordinating home care services, acting as a liaison between patients, physicians, hospital staff, and home health agencies. Responsibilities include contributing to discharge planning, documenting care activities, and educating staff and the community about home care and specialty programs. The role requires multitasking and prioritization skills, with experience in home care being advantageous.

• Assist in discharge planning and coordination of care activities.
• Participate in planning and educating about home health services and specialty programs.
• Serve as a liaison between internal teams and external partners.
• Document all care coordination efforts accurately.
• Contribute to sales and marketing efforts through education and outreach.

• Registered Nurse (RN) license required.
• Associate's degree in Nursing preferred, with 2-5 years of relevant experience; 5-10 years preferred for advanced roles.
• Experience in home care is a plus.
• Ability to multitask, prioritize, and adapt to travel between hospital locations.
