Hospice RN Case Manager

This position involves providing specialized nursing care and support to patients at the end of life, including assessing patient needs, developing and implementing personalized care plans, and collaborating with an interprofessional team to ensure quality, patient-centered care. The nurse acts as a case manager, coordinating all aspects of patient care, evaluating changes in condition, and communicating effectively with patients, families, and team members.

• Assess patient preferences, barriers, and needs across emotional, spiritual, cultural, and value-based domains.
• Develop, document, and communicate individualized care plans with clear goals, ensuring seamless care across settings.
• Promote respectful, equitable, and empathetic interactions with diverse populations, advocating for patient and family needs.
• Monitor patient conditions, update care plans as needed, and ensure evidence-based practices are followed to achieve optimal outcomes.
• Work in a safety-sensitive environment, maintaining constant alertness and safe practices.

• Minimum of a Nursing Diploma/Associate’s degree and RN licensure.
• At least 2 years of nursing experience; experience in home health or hospice preferred.
• Valid Driver’s License required.
• Preferred:
  Bachelor’s Degree in Nursing with 2-5 years of relevant experience.
