Role Overview
The position involves providing direct patient care in a 26-bed Acute Care Medicine unit, managing a diverse patient population with complex medical conditions, including occasional trach care. Responsibilities include assessment, diagnosis, planning, implementation, and evaluation of individualized care plans, promoting patient and family-centered care, educating on illness management, and ensuring safety and quality compliance. The role requires critical nursing skills to evaluate physical, psychosocial, and emotional needs, respecting patient preferences and cultural considerations.
Qualifications include a Nursing Diploma/Associate's degree, RN licensure, and preferred experience of two years with a Bachelor's degree in Nursing. The role emphasizes professional development, teamwork, and leadership within a multidisciplinary team led by clinical managers and supported by advanced practice nurses, case managers, and other health professionals.
Key Duties- Assess and evaluate patient needs using critical nursing skills.
- Develop and document individualized care plans with collaborative input.
- Educate patients and families on post-treatment care and medication management.
- Participate in activities promoting safety, quality, and compliance.
- Maintain communication across care teams to ensure continuity and optimal patient outcomes.
