Registered Nurse in the Transitional Care Unit

• Provide direct patient care, including assessment, planning, implementation, and evaluation within nursing standards
• Promote patient and family-centered care, educating on illness management, home care, and medication administration
• Develop and execute individualized care plans in collaboration with the healthcare team and patients' families
• Assess physical, psychosocial, and emotional needs, respecting patient preferences, values, and cultural considerations
• Participate in safety, quality, and regulatory compliance activities, and engage in professional development

• Education:
  
  Nursing Diploma/Associate's Degree;
  Bachelor's Degree preferred

• Experience:
  
  No prior experience required; 2+ years preferred
• Strong critical thinking, assessment, and clinical decision-making skills
• Ability to work in a fast-paced, multidisciplinary environment
