St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability.

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence‑based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.

Position Summary

St. Patrick Center, a ministry of Catholic Charities, is hiring a part‑time Shallow Subsidy Case Manager to join our Supportive Services for Veteran Families (SSVF) team. This case manager will utilize wrap‑around philosophy and techniques to guide veteran families on their journey to self‑sufficiency in the community. We are looking for a passionate case manager to assess client strengths, create strategies to achieve goals, and coach clients in learning skills for problem‑solving.

The case manager will coordinate individualized services, housing placement, and community involvement for veteran families. The core working hours are 8:00 am to 4:30 pm, Monday through Friday. This position works Monday‑Friday, a minimum of 20 hours per week.

Overview And Responsibilities
  • Make in‑home visits to observe indicators of client’s progress.
  • Conduct monthly interviews with clients to help develop and monitor an Individual Service Strategy.
  • Contribute to the ideas and processes of the Case Management (CM) team.
  • When needed assist with housing placement based on desires of clients.
  • Work with case management (CM) team to address long‑term financial stability needs of client.
  • Work directly with veterans who require therapeutic interventions.
  • Coach clients in learning skills for problem solving and long‑range planning.
  • Make appropriate referrals either to SPC Programs and Agency Partners.
  • Responsibility for completion and goal attainment of indicators outlined in grant and individual housing goals.
  • Assist in all activities consistent with daily duties of CM team to include home visits, child programming (if necessary), family crisis, ongoing housing stability.
  • Assist and complete special case management projects as directed by supervisor.
  • Responsible for completion of departmental goals of case management as outlined by grants.
  • Assess the client’s strengths, desires, and immediate/long term needs.
  • Develop or contact family and natural supports which exist in the community.
  • Collaborate with the Wrap‑Around Team that was developed.
  • Receive input from CM team members regarding employment, housing, mentoring, substance abuse and other immediate/long term needs.
  • Document all referrals or services provided to client into the ECM.
  • Complete daily and monthly departmental paperwork.
  • Update daily clients’ electronic files in ECM to include any types of contact with clients, services or any internal or external referrals.
  • Adhere to the strategies and goals outlined in Housing Plan.
  • Assist team members in addressing needs of client by providing transportation, clothing, furniture, food, hygiene kits, job referrals or any other necessary referral like mentoring, substance use, mental health and advanced training opportunities.
  • Coach clients to make solid, well‑informed decisions.
  • Refer client to additional resources to address other service needs, i.e., legal assistance, VA benefits, community substance use counseling, etc.
  • Research additional benefits available to enhance service being provided.
  • Engage client, family and natural supports in community setting when feasible and desired by client.
  • Participation in at least one Agency Sponsored Fundraising event per year.
  • Works with other Employment and Veteran Services Departmental staff.
  • Works with the staff in other SPC programs.
  • Networks with federal, state, and local agencies.
  • Other duties as assigned.
