Position SummaryThe Foster Care Case Manager (FCCM) is responsible for the safety and services of children and families in foster care. Tasks include facilitating Family Support Team meetings, developing individualized care plans, coordinating services across settings, overseeing providers working in the home, school and community, and documenting support‑related activities. The FCCM promotes effective utilization of support services to achieve permanency for children and families.
EssentialJob Functions
- Follow all contractual and agency policies, procedures, and deliver services seamlessly to children and families.
- Maintain a caseload of 12 to 15 children, subject to program capacity and agency needs.
- Provide full case management services to children and families in the Missouri foster and adoptive care system from assignment until case closure or transfer.
- Document all activities and maintain data entry in FACES and CSWIS accurately and promptly.
- Maintain face‑to‑face contact with each assigned child and family at least monthly; make telephone contacts with family support providers regularly to review progress.
- Evaluate and report the impact of services to team members and the judiciary; attend all court hearings; schedule, coordinate and facilitate FST meetings within prescribed time frames.
- Participate in the FCCM consortium on‑call schedule as employee on call for PCHAS on a rotating basis.
CM must possess a bachelor’s degree in social work, psychology, counseling, education, criminal justice or a related human services field. A master’s degree is preferred. Minimum of two (2) years of relevant case management experience.
Skills- Represent PCHAS to the professional community; strong verbal and written communication; ability to build and maintain positive community relationships with placement and treatment providers, schools and agency partners.
- Build rapport with children, parents and families; actively manage and resolve conflicts; excellent time management, organization, planning and assertive communication skills.
- Work a flexible schedule, including outside of normal working hours, and provide crisis intervention services as needed while maintaining confidentiality and HIPAA standards.
- Operate computer and standard office machines; navigate multiple data entry systems.
- Analyze variables and choose effective interventions; resolve problems, handle conflict and make effective decisions under pressure; demonstrate personal maturity and cultural competency.
Hold a valid driver’s license with a risk‑free driving record; willing to undergo initial and periodic background screenings; submit fingerprints; pass a pre‑employment drug screen and physical; maintain American Red Cross first aid and CPR certification at all times.

Compensation$43,000 – $47,000 yearly salary. $1,000 hiring bonus.
