POSITION SUMMARY STATEMENT

Regions Digestive Care is staffed by Advanced Practice Providers who are dedicated to clinic and hospital service, as well as GI physicians who have varying clinic rotations along with their procedural, and hospital schedules. GI Advanced Practice Providers manage a full patient caseload and are trusted experts in this specialized field.

They provide new patient assessment for patients with liver and general GI disease; ongoing chronic disease management for this patient population; and between-visit partnership with patients inthe management of their disease. These providers are an integral partof the GI team and the patient care they provide is foundational and essential in caring for the Health Partners patient population with GIand liver disease. Performs other duties as assigned.

MAJORJOB

DUTIES AND TASKS
1.

Obtains history and examines patient

  • Reviews patient’s records.
  • Discusses patient problem with patient.
  • Performs examination.
  • Documents in patient record.
  • 2.

    Orders appropriate labs and follow-up care

  • Documents orders in patient record.
  • Notifies appropriate personnel to carry out orders.
  • 3.

    Discusses with and instructs patient and family members regarding treatment and follow-up.

  • Documents in patient record.
  • Provides written instructions to patients (patient education).
  • 4.

    Communicates and follows up with patients in between clinic visits.

    10%

  • Review slab and test results.
  • Makes recommendations for treatment next steps and modifications.
  • May include Scheduled Telephone Visits.

    Interacts with other healthcare professionals.

  • Obtains pertinent patient information.
