GI Advanced Practice Provider — Digestive Care

Health Partners Digestive Care is seeking an Advanced Practice Provider to join a team of GI physicians and APPs. The role manages a full patient caseload, performs new patient assessments, chronic disease management, and collaborative care within a fast-paced GI setting.

The provider will work with hepatology and GI teams, coordinate with nurses, and participate in patient education, follow-up planning, and in-hospital rounds.
