Registered Nurse - Weekend Visits; RN Baylor Shift

Overview

Registered Nurse - Weekend Visits (RN) Baylor Shift at Brighton Hospice. The RN will provide care and services for a group of patients, visiting patients where they reside (private homes, Skilled Nursing Facilities, and assisted living facilities). This position is part of an interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources. The RN may work in a Case Management, Admissions, or Resource role.

• Provide patient care and services for assigned patients in their homes or care facilities as part of the hospice interdisciplinary team.
• Contribute expertise in psychosocial assessments and interventions; identify community resources to support patient and family needs.
• Work in a Case Management, Admissions, or Resource role as assigned.

• Current Registered Nurse (RN) licensure in good standing in the relevant state(s).
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership; strong teamwork skills.
• Ability to work closely with patients on a 1-to-1 basis while receiving support from the patient care team.
• Ability to work independently, communicating via phone, email, written notes; participate in meetings with healthcare professionals; understanding of hospice rules and regulations.
• Effective time management to plan visits, document care, attend meetings, and respond to urgent needs by prioritizing tasks.
• Ability to learn and use an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

• Weekend position, Friday, Saturday, and Sunday, 3 x 12 hour shifts (8am-8pm).
• Pay range: $85,000 – $91,000 annually.
