The Case Manager monitors and directs nursing staff to ensure resident care aligns with individual care plans, regulations, and quality standards.
- Reports to the Director and Assistant Director of Nursing, supervises RNs, LPNs, TMAs, and CANs.
- Coordinates care with interdisciplinary teams, residents, families, physicians, and external providers.
- Performs RN House Supervisor duties, including physician rounds, assessments, and clinical oversight.
- Manages the MDS process, updates care plans, and participates in care conferences.
- Provides pain management, advocates for resident rights, and promotes continuous quality improvement.
- Ensures compliance with regulations, maintains professionalism, and fosters resident dignity and privacy.
- Requires RN licensure, nursing experience, and knowledge of regulations and documentation processes.
Work hours: 40 hours/week, with holiday and weekend rotations.
Pay: $40.69 - $45.82/hr.
