Case Manager - Women's Shelter

Full-time, on-site in Saint Paul. Must reliably commute or relocate before starting employment. Physical setting: emergency shelter/transitional housing environment.
Schedule:
Day shift, Monday to Friday. Evenings 2–8 times each month, occasional weekends and holidays.
Union Gospel Mission Twin Cities (UGMTC) has provided emergency shelter, food, basic needs and wraparound programs for people facing homelessness, hunger and addiction since 1902. The Naomi Family Program offers residential housing, mental and chemical health support, job‑skills training, and faith development for women and children.
Responsibilities- Implement Naomi Family Program policy, procedures, and guidelines consistent with ethical standards.
- Assist in evaluating program effectiveness and achievement of objectives.
- Update participant progress in Case Worthy with accurate, timely case notes.
- Create a welcoming and safe environment for women and children; de‑escalate and resolve issues and conflicts.
- Identify and provide emergency crisis services; make immediate assessments and respond using accepted crisis methods.
- Conduct intake to gather physical, psychological, spiritual, financial, and social information; develop individualized goals and case plans.
- Use case‑management methodology to monitor goals through non‑therapeutic counseling and weekly meetings; provide backup coverage for other case managers when needed.
- Coordinate access and referrals to government and community resources: housing, food, clothing, school programs, vocational opportunities, life‑skills providers, mental health services.
- Track resident engagement in community resources.
- Document encounters, contracts, and maintain comprehensive participant files.
- Facilitate Basic Services classes, morning devotions, and support groups.
- Assist with social and recreational outings for participants.
- Collaborate with program team on case issues and progress; make program recommendations.
- Lead prayer and devotions; attend UGMTC sponsored spiritual enrichment activities.
- Cover front desk as needed.
- Address internal and external questions regarding UGMTC’s mission, vision, faith statement, and core values.
- Maintain cooperative relationships with other departments to ensure positive experiences and program success.
- Understand mandated reporting policies; attend regular training.
- Participate in seminars and training for case managers.
- Maintain knowledge of community resources (housing, transportation, childcare, vocational services).
- Use organization, planning, and administrative skills to manage workload and solve problems.
- Maintain professional boundaries with staff and participants.
- Implement program guidelines and offer suggestions for improvement.
- Cultivate a supportive, collaborative work environment to achieve participant outcomes.
- Perform other duties as assigned without notice.
- Bachelor’s degree in Human Services, Social Work, Psychology, or related field (or equivalent work experience).
- Minimum 2 years of case‑management experience.
- Experience working with individuals experiencing mental health issues, homelessness, poverty, domestic violence, or addiction in a residential shelter or transitional housing setting.
- Experience with County, Department of Human Services, Coordinated Entry, and permanent‑supportive housing resources preferred.
- Excellent driving record preferred.
- Valid driver’s license (Required).
- CPR certification (Preferred).
- U.S. work authorization (Required).
- Interpersonal and communication skills to work effectively with diverse constituencies and stages of recovery.
- Performed in a shelter/residential housing environment with no unusual physical demands.
- Typically worked in an interior office setting.
- Includes occasional pushing, pulling, reaching, crouching, stooping, walking, twisting, carrying.
- Moderate exposure to physical risk.
- Must be able to lift and carry 10–15 lbs.
