Summary:
The Waiver Case Manager provides comprehensive case management services for individuals that are aging and/or have been diagnosed with a disability. Typical case load includes individuals with complex medical diagnoses, mental health diagnoses, developmental disabilities, and those who are aging. This position requires meeting Disability Service program requirements as outlined by Ramsey County Human Services, Hennepin County Human Services, and Jewish Family and Children's Service.

Salary and Benefits :- Approximate Annual Salary $50,

- Work-life balance including 37.5 hour work week, 3 weeks of vacation, paid sick time, paid parental leave, holidays, flexible schedule, and the option to work up to 60% time from home
