ACCORD is seeking a dedicated Case Manager to support clients in the Dakota, Hennepin, and Washington County areas. The role includes assessing needs, creating care plans, coordinating services, and monitoring progress to ensure appropriate care.
Hybrid work arrangement with office presence in St. Paul and community/home visits. Full-time, non-exempt, 40 hours weekly. Compensation includes a $23–$25 hourly wage and a $1,500 new hire bonus paid quarterly.
