Community Engagement Coordinator
The Community Engagement Coordinator is responsible for building awareness of the Adult Day Care Center’s services through community outreach, relationship development, and referral coordination. This role serves as a key connection between the Adult Day Center, families, referral sources, and community partners to support enrollment growth and community presence.
Responsibilities- Develop and maintain professional relationships with case managers and care coordinators, healthcare providers, social service organizations, and community and faith‑based organizations.
- Promote Adult Day Care services through in‑person outreach, tours, and community events.
- Schedule and assist with facility tours for prospective participants and families.
- Support pre‑enrollment efforts by collecting and tracking referral and intake information.
- Follow up with families and referral sources regarding enrollment status.
- Represent the Adult Day Center in a professional and positive manner at all times.
- Assist with planning and participating in outreach events, health fairs, and presentations.
- Maintain accurate records of outreach activities, referrals, and contacts.
- Communicate regularly with leadership regarding outreach efforts and enrollment progress.
- Ensure all outreach activities comply with applicable regulations and company policies.
- Previous experience in community outreach, healthcare, social services, marketing, or a related field preferred.
- Strong interpersonal, communication, and relationship‑building skills.
- Friendly, professional, and confident demeanor.
- Self‑motivated with the ability to work independently and manage time effectively.
- Ability to pass all required background screenings.
- Basic computer skills and ability to document outreach activities.
- Combination of office‑based and community‑based work.
- Frequent interaction with community partners, families, and referral sources.
- May require attendance at community events outside standard business hours.
- Ability to travel locally for outreach and events.
- Ability to stand or walk for extended periods during events and tours.
- Professionalism and integrity.
- Organization and follow‑through.
