Description

The BrigAIDe Community Outreach Navigator works within Metropolitan Ministries’ Outreach Team to build relationships with individuals experiencing homelessness, providing services, support, guidance, and connecting them with resources and linkage to housing.

Job Functions

According to the mission and vision of Metropolitan Ministries, the role involves identifying unsheltered homeless individuals through canvassing, referrals from community partners, and direct street outreach activities. The candidate conducts housing assessments, monitors client enrollment and eligibility through monthly engagements, and maintains an ongoing caseload in the Homeless Management Information System (HMIS).

Essential Responsibilities
  • Assist with reducing barriers to housing and identify housing resources to decrease client homelessness duration.
  • Offer wrap‑around services in person, by phone, virtually, and at mobile sites.
  • Support clients in accessing resources and making referrals until they obtain shelter or permanent housing.
  • Enroll eligible clients in the Coordinated Entry (CE) supportive housing program.
  • Maintain an ongoing caseload in HMIS, ensuring all required documents are uploaded, assessments completed, and updates entered.
  • Maintain client records, daily activity logs, mileage logs, and other reports as directed.
  • Participate in Coordinated Entry meetings to identify barriers and collaborate with other service providers and community resources.
  • Facilitate access to continuum of care services including basic needs, housing assistance, substance abuse education, legal assistance, health information, and more.
  • Develop relationships through frequent outreach visits to encampments and other locations where homeless individuals congregate.
  • Research community resources that benefit individuals experiencing homelessness.
  • Respond to incoming calls on BrigAIDe outreach phone lines and administer services telephonically.
  • Input client services into HMIS daily.
  • Develop client goals and monitor progress via case management notes.
  • Provide one‑on‑one case management sessions for identified clients monthly or as needed.
  • Encourage and support client participation in activities and events that increase self‑sufficiency.
  • Ensure all case records are current by documenting notes in HMIS.
  • Advocate for individuals when they encounter barriers.
  • Drive a van/shuttle, conduct daily inspections, clean the vehicle, and maintain safety conditions.
  • Participate in professional development activities and required agency meetings.
  • Meet personal goals and contribute to team and organizational targets as outlined in KPI and annual plan.
Requirements

Education and Experience

Bachelor’s degree in social services or a related field, or an associate’s degree with relevant experience working with unhoused individuals, including volunteerism or direct service experience.
