• Provide the highest quality of care for home health patients in a professional and compassionate manner.
• Use assessment tools to manage the medical needs of a patient and identify other disciplines needed to maximize the patient’s outcome.
• Be a patient advocate and communicate with the primary Doctor, Clinical Team Manager, and assigned Registered Nurse as the patient’s condition requires.
• Contribute to a collaborative care team and support a positive patient experience.

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• 1 year nursing experience required; IV certification preferred.
• Ability to exercise initiative and independent judgment.
