Responsibilities
- Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.
- Provide case management to pregnant women and infants referred to the Healthy Start Program.
- Conduct face‑to‑face contact with clients either in the OB provider’s office or in the client’s residence.
- Provide one‑on‑one parenting education sessions.
- Provide breastfeeding and tobacco cessation education to individual clients.
- Document all client contact information in the Well Family System web‑based application.
- Provide referrals to community resources to supplement Healthy Start services.
- Experience in counseling techniques and strategies.
- Experience working with pregnant women and children.
- Problem‑solving skills related to advancing client self‑sufficiency.
- Ability to work well with others as a team.
- Computer skills: competency and experience.
- Experience in interviewing and assessing to determine the needs of the client.
- Knowledge of community resources.
- Valid Florida Driver License.
- At least 1 year experience working with high‑risk pregnant women and/or families.
- Must have reliable and dependable transportation every day for home visitation purposes.
- 1 to 3 years of experience working with families.
- 1 to 3 years home visiting experience.
- 1 to 3 years case management experience.
- Bilingual with fluency in English and Spanish.
