We’re on the hunt for a compassionate, organized, and dedicated Director of Social Services to join our amazing team. You’ll be the heartbeat behind our residents’ emotional and social well-being — guiding families, supporting transitions, and collaborating with an incredible interdisciplinary team. • Be a key player in ensuring our residents receive holistic, person-centered care

• Lead efforts to address the psychological, social, and physical needs of residents and their loved ones

• Collaborate with nursing, therapy, and administrative teams to create a true community of care

• Complete social assessments and gather important information at admission

• Maintain ongoing communication and support with residents’ families

• Assist our Admissions Coordinator by giving thoughtful, welcoming tours to prospective residents and families

• Complete pre-admission assessments to help residents transition smoothly into our facility

• A degree from an accredited school of social work

• Licensure in the state of practice (
  LSW, LCSW, or LICSW required)

• 2+ years of experience in long-term care preferred
